The Twin Brother
This is a method borrowed off soap operas, which can be used to replace a
deceased character. It's very simple - the new character is the identical
twin brother of the old one.
Using this method avoids the need for the entire game to stop for two hours
while a replacement character is created. Just rub out the name at the top
of the character sheet, and hey presto.
This doesn't solve all insertment issues, but it does make introducing the
new character easier.
"Jack... it can't be. You're dead!"
"No it's not Jack, it's... Jim."
At this point you can rule that people who were best friends with Jack
should feel equally at home with Jim and not require some kind of tiresome
"trust building" phase. If Jim's lucky he might even get a car, house or
even a wife thrown in.
Additional Note:- If you are a player this is always a good option since it
effectively allows you to keep the equipment your previous character had.

It's a good idea to always keep a few surplus NPCs handy to provide some
"prefab" PCs. This is a quick and easy technique although it can still warp
reality somewhat, as the NPC who has followed the party mutely around for
three years suddenly starts barking orders and generally throwing his weight
around.
If you want to be a bit more subtle you can keep the NPC in disguise. The
mangy old dog that follows the party around could always prove to be some
kind of wizard on an undercover research project...
